Support Coordinator Job in HealthFirst

The Care Coordinator is responsible for assisting the Care/Case Managers with non-clinical activities such as creating cases and events; providing telephonic outreach to members, providers and community-based organizations; handling member mailings; faxing clinical requests and Individual Health Care Plans on behalf of the care/case managers. The Care Coordinator is assigned to a care management call center or a specific department (such as Care Management, Utilization Management, Medicaid, etc.) and may have additional departmental responsibilities.

For Call Center: Receives incoming calls from members regarding benefit questions/ issues, care coordination, provision of educational materials, connecting to their Care Manager, and screening assessments to identify risk factors requiring clinical intervention. Confirms and documents accurate member information and call discussions in designated medical management systems.
For Care Management teams: Manages an assigned member caseload who require short term care coordination and health navigation; identifies issues pertaining to medical care access, navigation, and intervention; and monitors member progress in care management systems. Create medicals authorizations for services where approved and applicable.
Determines discharge planning, required follow-up calls, and whether certain issues need to be escalated.
Additional duties as assigned.

Minimum Qualifications:
High School diploma or GED.

Preferred Qualifications:
Basic Microsoft Word and Excel skills.
Language preferences – Spanish, Russian, French, Creole, Mandarin, Cantonese.
Experience in managed care or other area of the healthcare industry working in a Call Center environment or Care/Case Management Department.
Demonstrated ability to document calls into a computer system.
Knowledge of medical terminology.
