Weekend Home Care Call Support Job in Changing Seasons Home Care

*MUST LIVE IN THE SYRACUSE AREA * This is a remote position.

Education And Experience
High School Diploma required. Associates degree in Health Services or another related field preferred
Scheduling and customer service experience – in Home Care or another related field preferred
Familiarity with scheduling software
Expectations:
Requires availability from 5pm Friday -9am Monday
Answer phone calls at anytime day and night
Develops and maintains positive client and employee relationships
Communicates with clients and aides in a kind, positive, considerate and culturally sensitive manner.
Demonstrates an understanding of client and employee needs and wants and attempts to anticipate and meet client and employee needs.
Performs consistently with organization’s mission and philosophy.
Performs job responsibilities according to organization’s policies. Seeks clarification when needed.
Maintains HIPAA standards and confidentiality of PHI.
Accurately maintains and updates assigned client/employee schedules in scheduling system as necessary.
Makes all changes and adds appropriate notes daily to schedules for cancelled, changed or missed visits.
Communicates service changes to clients within the designated time frame. Communicates verbally with nurse regarding hospitalizations, expirations and occurrences.
Reviews alerts, missed visits reports through scheduling/telephony system daily. Contact employees who appear on the “missed in/out reports” daily.
Plans, organizes and provides clients with appropriate PCA/HHA services in a timely and efficient manner.
Ensures all shifts are filled to meet clients’ needs
Required 1-2 weekends per month and as needed
Working Conditions: This is a remote position. It may require occasional holiday work and must be available to come to the office for training.
